Out-Patient Treatment Guide

 

Should you need out-patient treatment or specialist care as an out-patient

The cost of any out-patient treatment should generally be paid by you at the time of receiving your treatment. It can then be recovered from us by completing, signing and submitting a claim form along with any supporting documents.

Whenever you visit a medical practitioner, dental practitioner, physician or specialist on an out-patient basis, take a Claim Form with you to help save time.

  •  

Complete the section that relates to you, then date and sign the Claim Form. Make sure that your medical practitioner, dental practitioner, physician or specialist completes all the relevant medical information required on the claim form in the sections indicated and then dates, signs and stamps the Claim Form.

Attach all original supporting documentation (don't forget to keep copies for yourself), invoices and receipts to the Claim Form (e.g. medical practitioner/physician invoices) and forward the form to ALC Health's Claims Team at :-

In the case of physiotherapy and complementary medicine, treatment is dependent on recommendation from a Medical Practitioner. The Medical Practitioner must therefore complete the medical certificate on the Claim Form.

Please remember that a separate Claim Form will be required for each person claiming and for each condition.

You can specify on the Claim Form the currency in which you wish to be paid, otherwise the benefit due to you will be paid in the currency of the invoice. Where a currency conversion is required, the rate will be that prevailing at the date of the invoice.


Important: If we do not receive a fully completed claim form or any supporting documents are not provided, there may be a delay in settling your claim. This is because we will need to revert to you or your medical provider for any information that might be missing.


High cost procedures

Where the cost of your out-patient treatment is likely to prove expensive, it may be possible in such cases to arrange for settlement of your costs to be made directly to your doctor / medical facility.

 We can try to arrange this service for you at least 5 days before the start of your treatment.

Please note that not all medical facilities will accept direct payment for us even where the cost of treatment is expensive. In these instances, you will still be required to settle the bill and submit a claim to us for reimbursement.
