Services, treatment and costs provided as part of any optical care (e.g. eye examinations, spectacles, contact lenses, etc.) should generally be paid by you. Please then submit the receipts along with a completed claim form to us for reimbursement.
As you will need to complete and submit an Optical Claim Form to reclaim eligible costs that you incur under your policy, please take a claim form with you on your first visit to your general practitioner, optician or specialist.
Important: If we do not receive a fully completed claim form or if any supporting documents are not provided (don't forget to keep copies for yourself), there may be a delay in settling your claim. This is because we will need to revert to you or your medical provider for any information that might be missing.
